FIELD DESCRIPTIONS:

FIELD   DESCRIPTION  
1. Date:   Enter the date the form is prepared.  
2. County / City / Parish:   Enter the E911 customer name.  
3. Prepared By:   Enter the name of the employee who prepares the form.  
4. Tel Co Name:   Enter the name of the telephone company.  
5. Serial Number:   Enter the appropriate serial number. (Begin the first AVR of each year with year, then the number of AVR sequence. For example, in 1993, the numbering begins with 92-001, 92-002, etc.)

6. Verification of:   Check the appropriate box to reflect that the AVR was due to:
  • A service order error.
  • An MSAG Ledger (include ledger #).
  • Other (explain in the comments section).
 
7. AVR Handled via Mail:   Check the appropriate box for the specific reason the AVR is prepared.  
8. AVR Handled via Telephone:   Enter the name of the person authorizing the update, as well as the date and time discussed.  
9. Existing MSAG Entry:   Enter only current MSAG information that is being changed or deleted:  
10. Directional:   Enter the directional indicator, N, S, E, W, NE, NW, SE or SW. If no direction, leave blank.  
11. Street Name:   Enter the name of the street including the standard thoroughfare designation (ST, etc.)  
12. State:   Enter the standard abbreviation for the state.  
13. Low Range:   Enter the lowest number in the street address range. If unnumbered, enter a line.  
14. High Range:   Enter the highest number in the street address range. If there is only one numbered address in the range, the low number will equal the high number. If the range is unnumbered, enter a line.  
15. O/E/B:   This describes the range.
  • O = Only odd numbers in the range
  • E = Only even numbers in the range
  • B = Both odd and even numbers in the range.
 
16. Community:   The name of the community in which the street and range exist.  
17. ESN:   Emergency Service Number.  
18. Exchange:   Entered by ICO, when appropriate. This data represents the code for the local exchange office which services the street address.  
19. Comments:   Enter pertinent information not included elsewhere on the form.

20. Response:   Check the appropriate box for your response:
  • Insert
  • Change
  • Delete
 
21. Desired MSAG Entry:   The E911 customer will enter the MSAG information to be input into the system. (See #9 for explanation of fields.)  
22. Comments:   Any additional information is entered here.  
23. Date Received:   The date the form is received by the E911 customer is entered here.  
24. Date returned:   If the form is returned by the E911 customer, the date is entered here. The form does not have to be returned unless the AVR is being handled via mail or if changes are required on a AVR handled via telephone.  
25. Authorized by:   The E911 customer's clerk's initials are entered here.

An example of the form prepared by the ICOs is shown on the following pages. It is the same form as the one used by INTRADO with the following exceptions:  
26. FOR ICO USE: (Shown only on ICOs form.)  
Received:   The ICO will enter the date the AVR is received.  
Referred to INTRADO:   The ICO will enter the date the AVR is referred (I.A.) to INTRADO, if appropriate.  
Clerk's Initials:   The ICO clerk who handles the AVR will enter his/her initials.  
27. FOR INTRADO USE:  
Received:   Enter the date the AVR is received from the ICO.  
Input to MSAG:   Enter the date the information is entered into the MSAG.  
Returned to ICO (I.A.):   Enter the date the form is returned to the ICO, if appropriate.  
Clerk's initials:   Enter the initials of the clerk who handled the form.
