• ICO NAME
• COUNTY NAME
• VOLUME SERIAL NUMBER identifies the volume and serial number of the tape.
• NUMBER and TYPE OF RECORDS SENT is the number of records sent and the type records is MSAG.
• ICO CONTACT NAME and NUMBER is the name and telephone number of the contact person to report any problems that may occur at the time the tape is loaded.
• ICO ADDRESS is only required if the tape should be returned.
• TN/MSAG COMPARE OR MSAG LOAD indicates purpose of tape.

Tapes should be mailed overnight to:
