Instructions for Completing the NXX Notification Form

ACTION CODE: Circle the Code that reflects the purpose of the form

A - ADD Add new information to an existing account, i.e. an additional NXX in an existing wire center

C - CHANGE Change information previously submitted, i.e. Contact Name, Address, etc.

N - NEW Notification of NXX(s) for a new county load of data

ICO NAME Enter the ICO company name.

ICO ADDRESS Enter the ICO company address for data correspondence.

ICO CONTACT NAME Enter the name of the person responsible for data issues.

CONTACT TELEPHONE NUMBER Enter the telephone number of the person responsible for data issues

TELCO ID/OCN Enter the four(4) digit numeric Telco ID or Operating Company number issued by BellCore

NENA CO. IDENTIFIER Enter the 3-5 character Alpha Company Code requested from NENA (National Emergency Number Association)

EFFECTIVE DATE Enter the anticipated date that you will begin sending updates. Please note that this form must be submitted no later than two(2) weeks prior to the NXX effective date for new NXXs.

SERVICE REQUEST FOR Circle the state where NXX is applicable.

NPA/NXXs: List all NPA/NXXs for the service area.

E911 TANDEM CLLI
(ALEC Coordinator will assist you in determining the correct tandem) Enter the CLLI code for the E911 tandem your NXXs will be routed to. (If tandem CLLI is not known, contact the BST AFIG.)
