LOAs – Letters of Authorization
LOA forms on this site are only to be used for services available in the FCC, BellSouth State GSST tariffs and interconnection agreements.
Letters of Authorization are required for multiple instances with BellSouth:
- A Facility Assignment Authorization (FAA) is required when a customer is utilizing another customer's facilities (ACTL / CFA). It also allows the CFA owner to issue ASRs in the behalf of his customer for any rearrangements. The customer that owns the facilities is referred to as the Host Customer.

- A Letter of Revocation of Carrier Facility Assignment is required when a customer revokes a previously sent FAA or LOA allowing a customer to utilize his CFA. The customer that owns the facilities is referred to as the Host Customer.

- A Letter for Ordering in Behalf of is required when a customer is acting as agent for another customer to initiate ordering in behalf of. The customer that ordering in behalf of is referred to as the Host Customer.

- A Letter for Carrier Identification Code Authorization allows the Host customer to translate the CIC code on his trunk group. The customer that is translating the CIC is referred to as the Host Customer.
